مهار فعالیت مهار کننده فعال کننده پلاسمینوژن نوع 1 سبب افزایش بازسازی سریع و پایدار خونسازی می شود
پیش آگهی بیماران تحت پیوند سلول های بنیادی خونساز ( HCST ) بستگی به بهبود سریع و پایدار خونسازی در طول زندگی دارد. فعال شدن مسیر فیبرینولیتیک سبب پیشبرد بازسازی خونسازی می شود، با این حال، نقش مهار کننده فعال کننده پلاسمینوژن - 1 ( PAI-1 ) ، یک تنظیم کننده منفی مسیر تجزیه فیبرین، هنوز روشن نشده است . ما دراینجا نشان می دهیم سلولهای استرومایی مغزاستخوان ، به ویژه استئوبلاست ها، در پاسخ به حذف سلول های مغز استخوان ( myeloablation ) که  بطورمنفی بازسازی خونسازی را در ریز محیط مغزاستخوان تنظیم می کنند، PAI-1 تولید می کنند. تابش دهی به تمام بدن در موش به طور چشمگیری سبب افزایش سطح بیان موضعی عوامل فیبرینولیتیک ، از جمله فعال کننده پلاسمینوژن بافتی (tPA) ، پلاسمین و PAI-1  می شود. اختلال ژنتیکی ژن PAI - 1 ، یا مهار دارویی فعالیت  PAI- 1، به طور قابل توجهی سبب بهبود مرگ و میر ناشی از myeloablation و پیشبرد بهبود سریع خونسازی پس ازپیوند سلول های بنیادی خونساز از طریق القاء عوامل پیشبرنده خونسازی می شود. توانایی مهار کننده PAI-1 در افزایش بازسازی خونسازی هنگامیکه موش های با نقص  tPA به عنوان دریافت کننده مورد استفاده قرار گرفتند، از بین رفت که این نشان می دهد PAI-1 ترمیم خونسازی به واسطه tPA را مهارمی کند. مهار کننده PAI-1 نه تنها تکثیر HSCs دهنده را در مرحله اولیه بازسازی تسریع می کند ، بلکه ازخونسازی طولانی مدت پشتیبانی می کند . نتایج ما نشان می دهد که ممانعت از فعالیت PAI- 1 می تواند یک روش درمانی برای تسهیل بازیابی سریع و پایدار خونسازی پس از پیوند سلول های بنیادی خونسازباشد .

Abstract

The prognosis of patients undergoing hematopoietic stem cell transplantation (HCST) depends on the rapid recovery and sustained life-long hematopoiesis. The activation of the fibrinolytic pathway promotes hematopoietic regeneration; however, the role of plasminogen activator inhibitor-1 (PAI-1), a negative regulator of the fibrinolytic pathway, has not yet been elucidated. We herein demonstrate that BM stromal cells, especially osteoblasts, produce PAI-1 in response to myeloablation, which negatively regulates the hematopoietic regeneration in the BM microenvironment. Total body irradiation in mice dramatically increased the local expression levels of fibrinolytic factors, including tissue-type plasminogen activator (tPA), plasmin and PAI-1. Genetic disruption of the PAI-1 gene, or pharmacological inhibition of PAI-1 activity, significantly improved the myeloablation-related mortality and promoted rapid hematopoietic recovery after HSCT through the induction of hematopoiesis-promoting factors. The ability of a PAI-1 inhibitor to enhance hematopoietic regeneration was abolished when tPA-deficient mice were used as recipients, thus indicating that PAI-1 represses tPA-dependent hematopoietic regeneration. The PAI-1 inhibitor not only accelerated the expansion of the donor HSCs during the early stage of regeneration, but also supported long-term hematopoiesis. Our results indicate that the inhibition of PAI-1 activity could be a therapeutic approach to facilitate the rapid recovery and sustained hematopoiesis after HSCT.
